WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND MEDIA RELEASE AGREEMENT
GBDCEI Inc. d/b/a Call My Ride
For participants in the Kidz-Preneur Academy – Kidz-Preneur Town
As consideration for allowing the child(ren) named below to participate in activities at Kidz-Preneur Town, a program of the GBDCEI Inc. (a 501(c)(3) nonprofit organization), I, the undersigned parent or legal guardian, acknowledge, understand, and agree as follows:
- Parental/Guardian Authorization
I affirm that I am the parent or legal guardian of the participant(s) listed below, or have obtained permission to act on their behalf for the purposes of this agreement.
- Nature of the Program
I understand that Kidz-Preneur Town is an educational simulation program that uses immersive roleplay to teach leadership, financial literacy, entrepreneurship, and real-world career concepts to youth ages 9–15. Activities include costume roleplay, teamwork, basic budgeting, field trips, and station-based tasks such as banking, grocery shopping, visiting a hospital, or running a business.
- Assumption of Risk
I acknowledge that participation in such activities may involve physical movement and social interaction, and while efforts are made to ensure a safe and supportive environment, minor accidents (e.g., bumps, slips, strains) or exposure to communicable illnesses (e.g., flu, cold) may occur. I voluntarily assume all such risks on behalf of the participant(s).
- Safety & Behavior Requirements
I understand and agree that:
– One adult must accompany every group of 10 children.
– Participants must wear light, flexible clothing (e.g., leggings or T-shirts) to accommodate costumes.
– No running or horseplay is permitted on the premises.
– Outside food or drinks are not allowed (except medical necessities).
– Participants and guardians must follow all rules and staff instructions.
– Any damage to props or costumes will be billed to the registering adult.
– GBDCEI reserves the right to dismiss any participant for behavior deemed unsafe or disruptive. No refunds will be issued in such cases. - Medical Emergencies & Insurance
In the event of a medical emergency, I authorize GBDCEI staff to seek appropriate emergency care. I understand that I am responsible for all associated medical costs and that GBDCEI does not provide health or accident insurance for program participants.
- Media Release
I grant GBDCEI Inc. the right to photograph and/or record video of the participant(s) during the program. Such media may be used for marketing, fundraising, and promotional purposes across platforms including print, web, and social media, without compensation.
- Waiver of Liability & Indemnification
I release and agree to hold harmless GBDCEI Inc., its directors, officers, staff, volunteers, sponsors, and partners from and against any and all claims for loss, injury, or damages resulting from participation in Kidz-Preneur Town. I further agree to indemnify and defend GBDCEI Inc. in the event of such claims.
- Binding Arbitration
Any unresolved dispute arising from this agreement shall be submitted to binding arbitration conducted within 25 miles of the event site, in accordance with the rules of the American Arbitration Association.
- Severability
If any portion of this agreement is held to be invalid or unenforceable, the remainder shall continue in full legal force and effect.
- Acknowledgment I have read, understood, and voluntarily agree to this waiver. I am of legal age and competent to sign this agreement on behalf of the participant(s).
